Symptoms
On : 12.2.10 version E-Business Suite
Following error occurs when clicking 'Submit a New Request' button for all the responsibilities.
STEPS

The issue can be reproduced at will with the following steps:
1. Login to E-Business Suite.
2. Select any responsibility.
3. Navigate to requests form and click on Submit Request button. Above mentioned error occurs.
