'Create Accounting' Program Did Not Transfer Payroll Subledger Lines to General Ledger

Symptoms
After running the Create Accounting concurrent program, it is found that some of the journal lines did not get transferred to General Ledger (GL) and remain stuck in the subledger (XLA) tables.
This can be confirmed by query:
Query results show:
GL_TRANSFER_DATE is null for all rows
ACCOUNTING_ENTRY_STATUS_CODE is F for all rows
This means the Create Accounting program ran in Final mode, but the transactions were not transferred to GL and are stuck in the subledger.
The issue can be reproduced at will with the following steps:
1. From HRMS Manager responsibility, run concurrent programs:
a. Payroll Run
b. Costing
c. Transfer to SLA
Navigate: Processes and Reports > Submit Processes and Reports > Single Request
2. From Global SLA/Payroll Manager responsibility:
Run Create Accounting concurrent process
Navigate: Processes and Reports > Submit Processes and Reports > Single Request
3. From General Ledger responsibility:
Check for journal entries with Payroll source and find that all the expected transactions were not transferred or imported.
Navigate: Journals > Enter > Find Journals
4. Run query above and find transactions stuck in the subledger (XLA) tables.
