Migrating from an earliest version of R12.2.X to a latest version of R12.2.X

Symptoms

While doing an upgrate to a latest version of Application R12.2.X, One of the Log workers is having the following error :

 ORA-00001: unique constraint (ZX.ZX_EXCEPTIONS_U2) violated ORA-06512: at line 3621

This is the complete error and log

Index re-created and restarted the patch. I got the same error:
