Material Not Getting Allocated In Work Order Even Though On Hand Is Available

Symptoms
When attempting to manually allocate material ,
Item "XXXX" has quantities Required = 30, Issued = 13, Allocated = 11, Open = 6, Available = 29, On Hand = 40.
In self-service apps, unable to manually request the full 6 open quantity due to error "Requested quantity must be less than or equal to the required quantity.".Able to request 1 quantity at a time successfully until the quantity changed to Open = 3, Allocated = 14.At that point,started to get the following error.

"An unexpected error was encountered: wip_picking_pt.allocate: ORA-01403: no data found."
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Navigate to Maintainance Super User
2. Work Orders > All > Query for existing WO and Open the Work order
3. Materials > Request All > Select check box for the Item > Enter Request Qty > Click Request
4. View error message
