Event Alerts on FND_USER, or FND Login/User Type Tables Cause Problems

Symptoms
(1) Upgraded from 11.5.9 to 11.5.10.2.
When logging into apps via OA_HTML/AppsLocalLogin.jsp or oa_servlets/AppsLogin all users will randomly get the error message:
Your session is no longer valid
You have an event alert on the FND_USER table and find that
when the triggers created by this alert are disabled then this error message
no longer exists when logging into the application.
(2) An event alert is created on FND_LOGIN_RESPONSIBILITIES to send an email
The Alert only fires when responsibilities are changed, but not when
a user initially logs into the application or selects a responsibility.
(3) Error received when logging into applications:
"Oracle error &ERRNO: &REASON has been detected in &ROUTINE"
