11.5.10.3: The FND_USER table is getting updated with the GUEST user name (Doc ID 434409.1)

This problem can occur on any platform.

Symptoms

When accessing a notification detail link from an email notification, a login page is displayed requesting the user to login with their own individual user name and password.  If the user is logged in already, the same session is used.   But the FND_USER table is not getting updated with the true user account, instead it is shown that ‘GUEST’ user was logged in.  While the notification moves to the next approver and the next approver logs in with his/her user_id and password, the FND_USER table shows that user ‘GUEST’ was logged in at the fnd_user.last_loging_date.
Also, the fnd_user tables shows some users (who use Applications only for the approval of notifications) have not logged in for years.
