FRM-40735: On-Error Trigger ORA-01001 FRM-42100 When Submitting Requisition or Purchase Order for Approval (Doc ID 459071.1)

Symptoms

After upgrade of Oracle EBS 11.5.10.2 database from 9.2.0.6 to 10.2.0.3., when attempting to submit Purchase Requisition for approval the following errors occur:

ERROR:
Forms message
FRM-40735: ON-ERROR trigger raised unhandled exception
ORA-01001.


Other messages appear

APP-PO-14056: User Exit #PO FIND_FORWARD returned error
Cause #PO FIND_FORWARD encountered an internal error. Parameters
FIND_FORWARD,3364,21,101,REQUISITION,PURCHASE,3433877,GLOBAL.forward_to_id,
and
Forms message
FRM-42100: Your connection to the Server was interrupted.

The same behavior occurs when trying to approve Purchase Orders

Steps to reproduce the issue
-----------------------------
The issue can be reproduced at will with the following steps:

1) Logon to Applications
2) Choose the Purchasing responsibility
3) Open Requisitions -> Requisitions form (POXRQERQ)
4) Enter Requisition details
5) Save & Click Approve
6) The "Approve Document" form opens
7) Check the "forward" checkbox
8) Enter %name% in the "Forward To" field
9) Tab
10) Get Error message:

FRM-40735: ON-ERROR TRIGGER RAISED UNHANDLED EXCEPTION ORA-01001.

The problem also occurs on FNDSCAUS form, Define Application User form when doing a partial search on Person field
