Service: APP-FND-01206: This Record Already Exists Error While Setting Up Service Activity Codes

When trying to create new Service Activity Code the following error message is received when saving  
the record:

"APP-FND-01206 This record already exists. You entered a duplicate value or
sequence of values that must be unique for every record"

Navigation steps:
For 11.5.8/11.5.9:
(N)Field Service Administrator -> Field Service Setup -> Installed Base -> Transaction Billing Type.
For 11.5.10 release
(N)Customer Support --> Setup --> Charges --> Service Activities and Billing Types
1) Enter transaction type in the Transaction Type field.
2) Tab to line category code, and choose Order (or any value).
3) Tab to next column and choose No update (or any value).
4) Tab to Description field and enter description.
5) Go to Related Billing Type block and choose Labor or any Material or Expense.
6) Go to Order Management Header & Lines type, click org name LOV and you will
   get the error message.
