Purchase/Credit Card Range Missing New Card Numbers (Doc ID 560239.1)

Symptoms

There are missing Card numbers in iPayment/Oracle Payments module.
Also found duplicate ranges which is causing orders with those specific purchase card numbers to fail.

Here is error that will be seen when trying to do an authorization with a card number that has a duplicate range:

ORA-01422: exact fetch returns more than requested number of rows
ORA-06512: at "APPS.IBY_PCARD_RECOGNITION_PKG", line 19
ORA-06512: at line 1
