APP-FND-01270 Error Generating Julian SYSDATE in Forms and REP-1416:'C_bal_segment_promptformula' In Reports

Symptoms
The problem occurs throughout the system, including, but not limited to: Creating journals, creating POs, entering invoices, querying on any of these forms etc. All responsibilities are affected and the problem may appear very intermittently.
Users get the following error within forms:
APP-FND-01270: Error generating Julian SYSDATE
Cause: Application Object Library could not generate the current database date
as a Julian value. (ORAERROR=1403)(ERROR=)
While running the Tax Reconciliation Report, the following error occurs:
MSG-01011: Starting FND SRWINIT
MSG-01235: Calling User exits
REP-1416: 'c_bal_segment_promptformula': User exit 'FND'. APP-FND-01270:
Error generating Julian SYSDATE.
