Could not Load Application Module Error During Candidate Registration (Creating New Account) (Doc ID 738048.1)

Symptoms

After upgrading to Release 12, the following error occurs when a candidate registers in iRecruitment (IRC):

server.IrcCandidatePersonalAccountAM) failed to checkout an application module due to the
following exception:

msg=JBO-29000: Unexpected exception caught: oracle.jbo.NoDefException, msg=JBO-25002:

STEPS
The issue can be reproduced at will with the following steps:
1. Go to the iRecruitment external site.
2. Press 'Register Today' button.
3. Enter details.
4. Press 'Submit' button.

Changes

Upgrade to release 12.
