Changing User Password Gets An Error Fnd_invalid_hash_mode (Doc ID 744059.1)

Symptoms

On 11.5.10.2 in Production:
When user is attempting to change login password, the following error occurs.

ERROR:
FND Invalid Hash mode detected for user_id = &USERID

STEPS
The issue can be reproduced at will with the following steps:
1. Go to Navigator Menu Edit> Preference > Change Password.
2. Enter Old Password and New Password/Re-enter password.
3. Press OK Button.
