Create New Activity or Asset Group Errors 'Could not create the Activity' or 'Insert Procedure failed with the following message' (Doc ID 974632.1)

Symptoms

There are two separate but related errors when creating either a new Activity or Asset Group

A. When attempting to to create Activity in the Activity Workbench, the following error occurs:

And can be reproduced at will with the following steps:
1. Responsibility: Enterprise Asset Management
2. (N) Activity Workbench => New
3. Try to save the form error message appears.

AND 

B. While trying to create a New Asset group, after entering the values, the following error occurs:

And can be reproduced at will with the following steps:
1. Responsibility: Enterprise Asset Management
2. (N) Activity Group => New
3. Try to save the form error message appears.
