On Debit Card Screen When Temporary Block Type Is Selected Fields Appearance Is Inconsistent. (Doc ID 2766288.1)

When attempting to select temporary block card type 
the following error occurs.

UI fields are in-consistent.

The issue can be reproduced at will with the following steps:
1. Login to OBDX application as retail user
2. Navigate to Accounts-->Current and Savings > Debit Cards and click on manage cards and navigate to block/hotlist card tab.
3. when temporary block is selected no fields are being shown,
  but when permanent block is and then specify the reason and whether replacement card required details are entered additional fields required are shown like address.
4.And if again temporary block select those fields are still shown, these fields didn't appear when user selected temporary block previously.
5.So users are confused with different details shown for temporary card in different scenarios.
