Symptoms

ACTUAL BEHAVIOR
------------------
When click 'add new' button in service request - form builder, 'Product name' field moves down and field above it loses its label name.

EXPECTED BEHAVIOR
-------------------
When click 'add new' button in service request - form builder, 'Product name' field  shouldn't scroll down and field above it should not lose its label name.

STEPS
---------------------
The issue can be reproduced at will with the following steps:
1. Login with administrator.
2. Navigate to service request – form builder
3. Now, create a form and fill the request parameters details
4. In request parameters, choose the one product name and now click on 'add new' button
5. now, fill the details and please check that field loses its label name and it gets displaced.
