Not Able to Add Date Field in Service Request Form Using Form Builder Template (Doc ID 3049742.1)

Symptoms

ACTUAL BEHAVIOR

While Creating a new Service Request/Editing an existing Service Request using Form Builder Template when a date field is selected under Build Your Form to add it from Predefined Element > Date, and saved, it does not show on the review screen but is available on confirm page. Once confirmed, this Service Request form when selected by Retail/Corporate user does not show added date field while raising this Service Request.

EXPECTED BEHAVIOR

While Creating a new Service Request/Editing an existing Service Request using Form Builder Template when a date field is selected under Build Your Form to add it from Predefined Element > Date and saved, it should show on the review screen. Once confirmed, this Service Request Form when selected by Retail/Corporate user should show added date field for picking date as optional/mandatory field while raising this Service Request.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Login as Admin user.
2. Use the toggle menu and navigate to Templates > Service Request-Form Builder.
3. Click on Create button to create a new Service Request Form or edit and existing form from the list.
4. Fill the required details and click on Next button to get to the Build Your Form and select Date from Predefined Elements. While saving the record the Date Element is not visible and is again available on the confirm screen.
5. Login as Retail User and use the toggle menu and navigate to Service Requests > Raise a New Request.
6. From the list select the newly created Service Request form with date field and observe the data field is not present.
