After Upgrading To Release 12, Approver Is Not Getting Requisition Line Details And Sometimes Bell Notifications And Approve Button Are Not Working Either

After upgrading to Release 12 (R12), Approver is not getting requisition line details and sometimes, Bell notification and approve button are not working either.
Steps to Reproduce:
1. Create a PR and submit for approval
2. Approver receives a bell notification
3. When trying to view the notification some times it is not accessible,
a) approve button is not working at times
b) Line details are missing
c) Bell notification does not open at times
