Unable to Trigger Quality Actions Using Multiple Characterstics in Single Action Rule (Doc ID 2613157.1)

Symptoms

ACTUAL BEHAVIOR
-------------------------
When the inspection results are entered and one result does not match the target, the action rules do not send out a notification nor create a quality issue

EXPECTED BEHAVIOR
--------------------------
Notifications should be sent and a quality issue should be created

STEPS
-------------------------
The issue can be reproduced at will with the following steps:

1) In the Quality Management work area, add an inspection plan, and add the Inspection Characteristics.
2) Under inspection characteristics, click Manage Action Rules.
3) Click Create Action Rule.
4) Add More than One Characterstic in IF Condition and add Quality Issue in DO Condition
5) Save and close the action rule.
6) Save the inspection plan
7) Enter the Inspection results and Save
8) Quality Actions not triggered
