OVM: Oracle VM Manager 3.x reports guest VMs stuck in Starting or Stopping status after Oracle VM Server (ovs-agent) upgrade (Doc ID 1477125.1)

Symptoms

After successfully starting guest VMs, Oracle VM Manager 3.x is unable to further manage guest Virtual Machines (VMs).
Oracle VM Manager 3.x reports an incorrect status of guest VMs.
Guest VMs may appear to be stuck in Starting or Stopping status although they have already successfully started or stopped on Oracle VM Servers.
Oracle VM Manager and Oracle VM Server logs denote messages such as the following:

-- Oracle VM Server: /var/log/messages

Changes

Default iptables firewall/rules are running on the Oracle VM Manager system.
The Oracle VM 3.x environment was recently upgraded to latest 3.1.1 errata.
Specifically, the ovs-agent package on Oracle VM Servers was updated to version ovs-agent-3.1.1-89.3 or higher.
