Goal

The following errors are shown in /var/log/messages:

kernel: Buffer I/O error on device dm-xx, logical block 1
kernel: Buffer I/O error on device dm-xx, logical block 2
kernel: Buffer I/O error on device dm-xx, logical block 6291440
kernel: Buffer I/O error on device dm-xx, logical block 6291440
kernel: Buffer I/O error on device dm-xx, logical block 6291454
kernel: Buffer I/O error on device dm-xx, logical block 6291454
kernel: Buffer I/O error on device dm-xx, logical block 0
kernel: Buffer I/O error on device dm-xx, logical block 0
kernel: Buffer I/O error on device dm-xx, logical block 1
kernel: Buffer I/O error on device dm-xx, logical block 6291455
kernel: Buffer I/O error on device dm-xx, logical block 6291455

When running lvscan, similar errors are shown as well:
