Unable to Access VM Console from Oracle VM Manager, ovs-agent.log Reports "Error: Device 51728 (vbd) could not be connected. Path closed or removed during hotplug add" (Doc ID 2334723.1)

Symptoms

A new VM is started from the Oracle VM Manager GUI, and it is in running state.

But it is not possible to connect to the VM console

AdminServer.log shows the following error:

'xm', 'block-attach', '0004fb0000060000b8a88c6c4f83ece4', 'file:/OVS/Repositories/0004fb0000030000b84eff6783172ef5/ISOs/OEL_5.11.iso', 'xvdb:cdrom', 'r'] failed (1): stderr: Error: Device 51728 (vbd) could not be connected. Path closed or removed during hotplug add: backend/vbd/4/51728 state: 6

 

And it was verified that the iso file exists.
