OLVM: Unable to Direct Attach Disk to a VM - Domain Already Contains a Disk With That Address (Doc ID 2673317.1)

Symptoms

On : Oracle Linux 7.8 version, Oracle Linux Virtualization Manager(OLVM) 4.3.6

When hot-adding a new 7th virtioSCSI disk (iSCSI or FC LUN) to a running Virtual Machine, OLVM will create the disk but will fail to attach it to the running VM.
OLVM Web Interface will not give an error, but the disk will stay with a Red Icon and will not be attached to the VM.
