Oracle Linux: OCFS2 Cluster Stack is Showing Offline and OCFS2 Filesystem not Mounted on Reboot (Doc ID 2703295.1)

Symptoms

After configuring a new cluster, the cluster stack shows as offline with ocfs2 volumes not mounted after boot.
A manual start was then attempted by running "/sbin/o2cb.init online" and then "mount -a" which helped to initialize and mount the disks.

However dmesg and the messages file showed the following errors:
