Symptoms

Storage paths not automatically recovered during IBM 2145 SAN controller upgrade.

After the upgrades the paths show as failed and a manual scan was required to recover the paths

 

# echo "- - -" > /sys/class/scsi_host/hostX/scan

 

Changes

Updated the IBM 2145 SAN Firmware
