Oracle Linux: System running UEKR6 Fails to Detect QLogic FC HBA - Error: qla2xxx: probe of 0000:0f:00.1 failed with error -12

Symptoms
An Oracle Linux 7 (OL7.9) UEKR5 (4.14.35) system is running on HPE ProLiant DL380 Gen10 Plus server fitted with QLogic Fiber Channel HBA Cards.
The system is updated to use UEKR6 e.g. kernel-uek-5.4.17-2011.6.2.el7uek.x86_64.
Booting into the newly installed UEKR6 kernel, the system fails to detect the QLogic FC HBA cards.
The HPE iLO keyboard and mouse are also unresponsive.
The following messages are reported to the system log (/var/log/messages)
Limited further testing identified the following affected and unaffected RHEL, RHCK and UEK kernels:
Affected:
- UEKR6QU0 kernel-uek-5.4.17-2011-*
- UEKR6QU1 kernel-uek-5.4.17-2036.*
- RHEL 8.2 kernel-4.18.0-193*.el8
Unaffected:
- RHEL 7.9 kernel-3.10.0-1160.el7
- RHCK 7.9 kernel-3.10.0-1160.el7
- RHEL 8.3 kernel-4.18.0-234.el8 onward
- UEKR5QU* kernel-uek-4.14*

In this instance, the system was updated with UEKR6QU1 kernel-uek-5.4.17-2011.6.2.el7uek.x86_64
