Symptoms

It may be noticed that Oracle Linux system with QLogic FC(Fibre Channel) HBAs does not perform multipath failover properly in a scheduled or unexpected storage outage which further incurs IO errors or even node evictions.

The errors like below can be observed in /var/log/messages and/or vmcore-dmesg.txt,

This is normally a sign of QLogic FC HBA's firmware hang.
