Life Event: When Employee Cancels Life Event Document They Uploaded Doesn't Get Backed Out

From Employee Landing page
1) Click on Benefit Details tile
2) Click on Life Events
3) Click on Birth
4) Input event date and click on Start Life Event
5) Continue thru steps and after load document then hit 'cancel' button
After this when EE goes back in to Life Events they are able to start over however the document they approved is in the queue for Benefits Administrator to approve which they can't since the event was cancelled by EE. When Benefits Administrator is trying to approve the document they get the error below:
SecurityException:ActionItem 'EF_nnnn' does not exist (263,90).
