Symptoms
When running Create ACA XML Files, the system is not correctly populating the header lines in the Request files.
STEPS

The issue can be reproduced at will with the following steps:
1. Run the Create ACA XML Files process (Navigation: Benefits -> ACA Annual Processing -> Transmittal and Forms -> Create ACA XML Files).
2. Review the contents of the Request files.
