Symptoms

When uploading a file attachment, after the user clicks the Upload button, it returns to the file form page again instead of closing

It is expected the window to close

The issue can be reproduced at will with the following steps:

Self services > student center > click Milestones > click one Milestone form the list > click Add button to attach file
