Matriculating Student in Second Program Results in Error: 'The Application Activation Process did not complete. (14200,95)'

Symptoms
When attempting to matriculate a student in a second program - after they have completed one program - using the same EMPLID (but different application ID's), the following error occurs and is displayed on the page after the Create Program button is clicked.
The issue can be reproduced with the following steps:
- Navigate to Records and Enrollment > Career and Program Information > Student Program/Plan.
- Search for an EMPLID e.g. AA0001.
- Make sure there is a completed program for the student. If none, create an application via Student Admissions > Application Entry > Add Application for Academic Program, e.g. LAU
- Matriculate the student.
- Check the Student Career Number on Student Program/Plan stack.
- If the Student Career Number of the Completed Program = 1, create another application for a different program, e.g AA, and make sure the Student Career
Number is lower than the completed program, ie 0. - Try to matriculate the application with Student Career Number = 0 - click on Create Program button on the Application Program Data tab, and note the error displayed on the page.
