Symptoms
On the Update Individual Dependent/Beneficiary Information page, Address Labels Show as "Group Box" in Small Form Factor.
STEPS

The issue can be reproduced at will with the following steps:
1. Using Small Form Factor (Navigation: Employee Self Service > Benefit Details > Dependent/Beneficiary Info).
2. Click on Dependent.
3. Review Address Labels.
