Symptoms
When creating or accessing a form, the following error is seen on the PIA page.

The issue can be reproduced at will with the following steps:
1. Log into PIA, and navigate to Enterprise Components > Forms > Design Form.
2. Create a form.
3. Observe the error shown above.
