When using Fluid Form builder wizard to create a new form we find that the new fields on our form do not display after we add them attempt to fill in or preview the form.

The issue can be reproduced at will with the following steps:
1.  Click the Design Form in FAB tile and Populate a new name for your form without any blanks in it, and click the ADD button to create  a new form.

2.  Populate the Label field (also without any blanks) and descr and click next.

3.  From the left grab a text box and drag it to the +signed area on your page.   On the right give your field a label and a descr and mark it 'required'.

4.  The next tab is instruction.   Leave this blank it's optional.   Click next.

5.  The next tab is Approval and should default in as 'Do not lock down".   You do not need an approver, it's optional.    Click next

6.  The next tab is Security.   Assign a role such as 'CS Administrator' for this test under
Security Authorizations
Reporting Authorizations.

7.  Click Save and Click Activate.

8.  From the Home page Click the 'My Forms' tile and "Fill out new form" option on the left to see your form but notice the field you added isn't there.
