Provisioning new environment in PCM failed while deploying midtier.

Following error captured in pt_tools_deployment.log file :

The issue can be reproduced at will with the following steps:
1. Log in to cloud manager PIA
2. Subscribe to CS application DPK
3. Try to deploy a new environment from environment tile
4. It fails while deploying midtier
