EAP: "Reference not found" is Displayed as File Name When Opening Supplier Attachment page

Symptoms
Supplier Attachments page shows 'reference not found' and no row is created in PS_PV_ATTACHMENTS.
Steps:
1) Go to Suppliers >> Supplier Information >> Add/Update >> Supplier.
2) Open Supplier information.
3) Go to Identifying Information tab.
4) Click Attachments link:
- a specific row is showing 'Reference Not Found' as the file name.
