Self Service Name Format Changes gives when page field configuration enabled for Name Change component.

Steps to reproduce the issue:

1.Configure Page field configuration for name change component, hide first name
Enterprise Components > Page Field Configuration > Page Field Configurator
2. Employee self service > Personal details > Name change
