Employee Benefit Plan Incorrect on HIPAA 834 File

Symptoms
The Benefit Plan for one employee incorrect on HIPAA EDI 834 file.
Steps to reproduce the issue:
1. Run the Refresh Benefits Snapshot process (Navigation > Benefits > Interface with Providers > Refresh Benefits Snapshot).
2. Check the Review Snapshot Information (Navigation: Benefits > Interface with Providers > Review Snapshot Information) to see that the employee and 2 dependents all have the same plan.
3. Run the HIPAA EDI 834 file for the provider (Navigation: Benefits > Interface with Providers > Create HIPAA EDI 834 File).
4. Check the file to see that plans reported for one or more employees are incorrect.
