Symptoms

While selecting Academic Institution from the Request Transcript Report page, field 'Report Request Nbr' gets defaulted to 000000000 and it's greyed out.

The issue can be reproduced at will with the following steps:

1. Navigate to: Main Menu > Records and Enrolment > Transcripts > Request Transcript Report
2. Select 'Academic Institution' using prompt
3. Noticed that field 'Report Request Nbr' gets defaulted to 000000000 and it's greyed out
