E-PIA: After Upgrating to PT 8.61.01 - 8.61.03 and Using a List, if a Radio Button at Position 2 or Later is Selected, the Button is Automatically De-selected (Doc ID 3034718.1)

Site upgraded from PeopleTools (PT) 8.59.11 to PT 8.61.03.
After doing so, when a list is used on a PIA page, the first radio button can be selected, but not the subsequent ones (items 2 and later).
An example of this is found on HCM ONBOARDING's Manage Questionnaires page.

Replication steps:

1. Log into PIA, and navigate to:  Enterprise Components > Questionnaire Framework > Questionnaire Setup > Manage Questionnaires.

2. Click on the ONBOARDING entry to edit it.

3. Navigate through the first two activities to the third one (e.g. "Add Questions"). 

4. Select the first item's checkboxes. Then, press/select the second row's radio button.

5. Observe that the user is returned to the top of the page, and the radio button selected in step 4 is not selected.
