New Status Creation Failed With SBL-DAT-00393 Error Under Site Management->Subjects in CTMS Application

Symptoms
When user add a new Status under Site Management -> Subjects it fails with below error.
The issue can be reproduced at will with the following steps:
1. Login to application
2. Navigate to Site map
3. Select site management screen
4. Drilldown on Site#
5. Select "Subjects" tab
6. Enter new record with below details
Subject Id: Test1
Encountered Date: 11/10/2023
Open Status then try to create record by clicking "+" and enter "Status" as "Screened" then save the record
Result:
An end of file error has occurred.
Please continue or ask your systems administrator to check your application configuration if the problem persists.(SBL-DAT-00393).
