NFS: Why do I get "stale NFS file handle" errors?

Symptoms
Getting "Stale NFS Handle" errors:
<date+timestamp> <nfs_client_hostname> nfs: [ID <id> kern.info] NOTICE: [NFS4][Server: <nfs_server_hostname>][Mntpt: /mnt]NFS op OP_GETATTR got error NFS4ERR_STALE causing recovery action NR_STALE.
<date+timestamp> <nfs_client_hostname> nfs: [ID <id> kern.info] NOTICE: [NFS4][Server: <nfs_server_hostname>][Mntpt: /mnt]File <filename> (rnode_pt: <rnode_pt>) was closed due to NFS recovery error on server <nfs_server_hostname>(failed to recover from NFS4ERR_STALE NFS4ERR_STALE)
