During a disk replacement or pool expansion task, "zpool add" or "zpool replace" may fail with the following symptoms

# zpool replace rpool c4t5000C50016A0599Bd0s0 c4t5000C50004481647d0s0
cannot open '/dev/dsk/c4t5000C50004481647d0s0': I/O error

# zpool add tank c4t5000C50016A0588Bd0s0 c4t5000C50004481647d0s0
cannot open '/dev/dsk/c4t5000C50004481647d0s0': I/O error


Note: In this situation slices are being used rather than giving ZFS the entire disk
