Symptoms
1) After power-cycle, binding of an I/O domain fails with error.
Or
2) After a restore of the system wide LDom configuration via "ldm init-system -r -i 'config_name.xml'

| 1) Remove the device from the I/O domain, re-add the device, and re-bind the domain. |
| 2) PCIe endpoints cannot coexist on an entire PCIe bus allocation. If the full bus is allocated to the IO domain then all endpoints must be removed. |
