Solaris Volume Manager (SVM) System Hangs When Root is Encapsulated and Logging Enabled and >50% of State Database Replicas Metadb are Lost (Doc ID 1472328.1)

Symptoms

After the system was patched and rebooted the system panic'd and rebooted itself.

The prompted to boot into single user mode due to:
Installation of patch set complete. PLEASE REBOOT THE SYSTEM.

Install log files written :
 /var/sadm/install_data/s10s_rec_patchset_short_2012.05.15_20.31.11.log
 /var/sadm/install_data/s10s_rec_patchset_verbose_2012.05.15_20.31.11.log
