Solaris Cluster Database Start Shows The Database as Being in an "OFFLINE" State (Doc ID 1541105.1)

Symptoms

After installing and configuring Oracle Solaris Cluster, ASM and Oracle Grid Infrastructure (now required for ASM 11.2.x+ onwards ) the following problem can occur.

Oracle Solaris Cluster could start the database fine using clrg online -eM db-rg, however after about 10 mins the database would then go down. 

The /var/adm/messages show :

 

The database startup via the Solaris Cluster (using crsctl), starts up the database, however the clusterware resource shows it's state as being  "OFFLINE".  
This causes the Solaris Cluster resource to run into the startup timeout for the database resource and thus enter the FAILED state.
