Symptoms

Customer may report a power supply issue.

Checking the environmentals using the following commands showenvironment (ALOM) or show -d properties -level all /SYS/<PS#> (ILOM), in the section of the power supplies you can see a failed power supply:

There's also an amber LED active for a power supply fault, also from showenvironment

 Occasionally, you may also find entries in /var/adm/messages file with following:

Apr 19 06:09:30 <host> SC Alert: [ID 325448 daemon.alert] IPMI | critical: ID = d : 04/19/2012 : 12:48:26 : Voltage : /PS2/AC_POK : State Deasserted
Apr 19 06:09:30 <host> SC Alert: [ID 769395 daemon.alert] IPMI | critical: ID = e : 04/19/2012 : 12:48:27 : Power Supply : /PS2/FAIL : State Asserted

 
and if needed the status can also be checked from the SP (ilom shell):

-> show -d properties -level all /SYS/<PS#>, where X is the suspect power supply
-> show faulty
-> show /SP/logs/event/list

Changes

If Power Circuit or Power Cord is interrupted from the source to the server, System Admins may generate Service Requests for investigation.
