Oracle Exalytics EM 12c Monitoring EPM Shows Errors "The percentage of requests that resulted in errors is %" and "File Not Found" (Doc ID 1622517.1)

Symptoms

When using Enterprise Manager (EM) 12c Cloud Control with Essbase/Enterprise Performance Management (EPM) on Exalytics, the ohs_component shows multiple log messages like "file does not exist." The EM12c error rate shows messages like "The percentage of requests that resulted in errors is %".  There does not appear to be any impact on EPM, Essbase or performance.
