Symptoms

Disk c1t1d0 has been replaced, after all Solaris Volume Manager (SVM) metadevices on it were detached and cleared. Most slices of drive have been initiated as metadevices, but an error is seen when attempting to run metainit on d21 (c1t1d0s1):
