Solaris Cluster with EMC Quorum Device/Disk Shows 'lost operational quorum' panic or Registering Quorum Fails with 'error 13' (Doc ID 2024874.1)

Symptoms

In this situation a Solaris Cluster is used with EMC storage as shared devices.

When rebooting one node of a two node cluster the other will panic with a "Cluster lost operational quorum" message.

New installation or introduction of new EMC LUNs one of which is used as the quorum device.
