zpool status output reports incorrect devices for vdevs

Symptoms
If the described issue occurs, the zpool status output will show incorrect devices for vdevs. If the zpool is an rpool, the system may become unbootable.
Here is a sample output showing the problem:

The issue can result from a device path change. zpool status output may continue to show the old pathname for the vdev.
